You can contact us without giving your name. Our complaints and feedback form can be submitted anonymously if you prefer.

We do not collect sensitive health information through this website, and we ask that you do not include detailed health information in website forms. We will gather any information needed to support you safely once we begin working together, with your consent and through secure means.

How we use your information

We use the information you provide only to:

Respond to your enquiry
Arrange an initial conversation or service
Respond to and resolve any complaint or feedback

We do not use your information for marketing without your consent, and we never sell your personal information to anyone.

How your information is sent and stored

Website form submissions are sent to us by email to hello@azoros.com.au.

We do not store website form submissions in any online database connected to this website. Your message reaches us as an email, and we keep it only within our secure email account for as long as we need it to respond to you and meet our record-keeping obligations as an NDIS provider.

Our website forms are processed by a third-party form delivery service solely to pass your message to our email. That service does not use your information for its own purposes.

How long we keep it

We keep your information only for as long as it is needed for the purpose you gave it to us, and to meet our legal and NDIS record-keeping obligations. When it is no longer needed, we delete it.

Your rights

You have the right to:

Ask us what personal information we hold about you
Ask us to correct information that is wrong or out of date
Ask us to delete information we no longer need to keep
Make a complaint about how we have handled your information
